Prof. Dr. Chakradhar Maddela – A Lifelong Commitment to Rural Pediatric and Neonatal Health

Prof. Dr. Chakradhar Maddela, a 30-year-old medical practitioner, graduated in medicine from Osmania Medical College, Osmania University, Hyderabad, specialized in paediatrics and super-specialized in neonatal medicine from Cardiff University and Southampton University, UK, in echo- echo-echocardiology & paediatric-neonatal cardiology. He is also trained in neonatal neurology, diabetology, pediatric emergency medicine, pediatric intensive care medicine, EEG management, invasive and non-invasive ventilation, and health research. Published research articles in indexed journals, researchgate.net, various online medical apps like Curofy, doxplexus, medshar, academiaedu, own YouTube channel, slide-share, Facebook, and many more. India is living in villages. The majority of the Indian population is women and children, and they are a high-risk risk vulnerable population. He had migrated from a metropolitan city to a remote rural area, Metpally, at people’s request to serve the children. It was a remote place in Telangana, 80 km away from the district headquarters, with limited access to health care for acute illnesses in children, and its associated high mortality and morbidity before 1995.

He had started a first paediatric hospital with round-the-clock emergency services. They established 1st neonatal intensive care unit and paediatric emergency services. Gradually, the unit upgraded with modern equipment for disease evaluation and efficient management, like invasive and non-invasive ventilation, and surfactant therapy for respiratory distress of newborn babies. For the first time in the region, he brought bedside echo-cardiology for hemodynamic evaluation of neonates, infants, children, and extended echo-cardiography services to adult patients as well. Hemodynamic evaluation is essential for grading the illness, detection of shock, and its effective management.

Echo cardiac facility helped their patients in early detection, medical management, and timely referral for corrective cardiac surgery of congenital heart diseases. He had introduced bedside neuro-sonogram and Trans cranial Doppler evaluation, which helped their infants in early detection and severity assessment of devastating neurological conditions, and their early management, introduced therapeutic hypothermia for post-asphyxiated babies who did not cry and initiate breathing at birth, EEG, and laboratory. They saved countless infants and children who were brought in the terminal stage of illness, especially those belonging to poor socio-economic strata, and contributed to improving infant mortality and under-five children mortality.

Regionally created the concept of ‘well baby’ among parents and in the local community by conducting countless well baby shows and delivering health education. A significant number of children in India and globally are deaf and blind, and it is crucial for early detection for timely referral, and management. He had started a neonatal screening program for all visiting newborn babies for eye vision, hearing, and thyroid status. Telangana is the diabetes capital of the country, and rural people have less access to specialized diabetic care management. He realized this scenario and trained in diabetology much earlier (a decade before it was available at the district level) and started echo-cardio-diabetic services to rural people. He had worked with NGOs like Rotary club, Lions club, Sathyasai Seva Samithi, local professional association, officials, and social welfare hostels. With senior colleagues’ support, he started Metpally Dr’s Association & later transformed into IMA KORUTLA-METPALLY BR. He had participated and taken a leadership role regionally and implemented programmes like poliomyelitis eradication/pulse polio immunization, deworming children, anaemia and malnutrition detection and management, free vaccination for >5 years for all infants in the private set-up.

Performed family planning surgeries in government health care centres – tubectomy and no-scalpel vasectomies for eligible couples and contributed towards population control. Started proper and better referral care and follow-up services for needy patients and was attached to a district medical teaching institute. He has been continuing as a medical teacher for undergraduates and postgraduates and is involved in teaching, research, and skills development. He is a member of professional bodies (national) like IMA, IAP, NNFI (Executive board member NNF Telangana ), PCSI, SFM, (INTERNATIONAL) RCPCH, AAP, NBS, ESC, ESPNIC, & ISUOG.
